What is TMD?

The temporomandibular joint or TMJ acts like a sliding hinge, connecting your jawbone to your skull. TMD is the dysfunction of the joint which can lead to pain and discomfort. Patients often endure a frustrating and extensive search for answers and relief of their symptoms. 

Symptoms may include but are not limited to:
  • Chronic headaches
  • Ear fullness or ear pain
  • Clicking popping of the jaw
  • Clenching/grinding of the teeth
  • Limited range of motion in the jaw
  • Pain when chewing
  • Facial pain
  • Neck pain or stiffness
  • Ringing in the ears
  • Brain fog
  • Eye strain
Cause:

TMD can be caused by trauma to the area due to injuries such as a car accident, whiplash, a blow to the head or neck, the teeth not meeting properly or previous orthodontic treatment. The teeth can appear to be perfectly straight, but this can be deceiving. The patient may still have a problem because the alignment (occlusion) does not match with the correct jaw joint or muscle position. While general dentists can recognize signs of TMD, such as grinding down the teeth, teeth chipping and cracked teeth, they unfortunately are not trained in treating TMJ.

While they may suggest a night guard, this can protect the teeth but does not correct the misalignment in the jaw or teeth that must be addressed. Dr. Maryam Bakhtiyari is specialized to look for the appropriate jaw position. This is especially important before orthodontic treatment.

Evaluation/Diagnosis: 

During your consultation a CBCT scan, a comprehensive head and neck evaluation and health history will be conducted and reviewed by Dr. Maryam to properly diagnose the underlying problem contributing to the symptoms. Patients leave with a clear understanding of their anatomy and how it is contributing to their ongoing symptoms. 

Treatment

Our treatment begins with custom-fabricated orthotic splints designed to stabilize the temporomandibular (TM) joints in their optimal position. By gently guiding the jaw into a healthier alignment, these splints help alleviate chronic and often debilitating symptoms associated with TMJ disorders.

In some cases, orthodontic treatment may follow to further enhance stability and long-term results. Additionally, we incorporate complementary therapies such as therapeutic massage, spray-and-stretch techniques, osteopathy, and targeted jaw exercises. We also utilize BLA®, a specialized facial yoga tool, to support muscle balance and relaxation.
